At the sharp end of diabetes care

Insulin injection treatment for diabetes sufferers would be much more effective if medical workers and patients did it properly. Shan Juan reports.

Wang Bao'an, 62, has been living with type 2 diabetes for nearly 16 years and injects insulin four times a day to help deal with weak eyesight, itching skin and kidney problems. "I would have been better off if I had started insulin injection therapy earlier," Wang, who is from Shaanxi province, says.

The overweight man has been taking insulin injections for nearly six years, 10 years after he was diagnosed with the chronic disease.

Currently, the nation has an estimated 92.4 million diabetics, about 60 percent of whom have not even been diagnosed, a nationwide epidemiology study conducted by the Chinese Diabetes Society (CDS) shows.

About 35 percent of those who need insulin injection therapy reject the regime, citing reasons like fearing dependence on insulin injections and having trouble ensuring regular injections, according to the study.

"This seriously affects the efficacy of diabetes treatment and the life quality of sufferers," says Ji Linong, head of the CDS, at a diabetes care and management seminar held in Nanjing, Jiangsu province, on Saturday and Sunday, adding that the annual medical expenditure related to diabetes is approximately $25 billion, making up about 13 percent of the total on the Chinese mainland.

"I was recommended by my doctor to start insulin injections after I had been living with the condition for seven years, but I simply ignored (the advice). I found the injections too troublesome, particularly on business trips," Wang says.

In most Western countries, diabetics start taking insulin injections about five to six years after diagnosis, according to the International Diabetes Federation (IDF).

Without timely insulin injection treatment blood glucose levels are difficult to control and there is a greater risk of developing serious complications like kidney problems, a stroke, or even limb amputation.

The only way to reduce or avert these conditions is to control blood glucose levels, says Guo Xiaohui, deputy director of CDS. However, of all the diabetics on the mainland, less than 27 percent have their blood glucose under control, official statistics shows.

Even among those who are taking insulin injections, just 37 percent have well-controlled blood glucose levels, Ji says, citing results from previous studies.

"Poor insulin injections by the patients and medical workers, particularly at the grassroots level, is one of the major factors for this," he explains.

Wang too had problems injecting himself with insulin. Using an insulin pen, widely used worldwide, Wang kept reusing it until it was bent.

"I thought it was OK as the needle was for my use and other patients did it too," he says.

In fact, this caused fat hypertrophy at the injection sites, affecting the accurate intake of insulin.

A regional survey polling nearly 500 patients at 200 diabetes care centers in 10 Chinese cities has found insulin injection problems, such as reuse of the needle, improper injecting skills, and choosing an inappropriate injection site.

Just 22 percent knew the right time and site for injection and how to properly rotate the sites, according to a survey conduced by CDS from September 2008 to June 2009.

About 40 percent of patients forgot to do their insulin injections from time to time and most of the insulin injectors reused the needle largely for economic reasons, the survey showed.

Only 10 percent of the injectors used new needles for each injection, as recommended by health professionals.

Improper use caused pain and bleeding at the injection site, insulin leakage at the injection site, clogging of the devices, and skin problems at injection sites.

Guo emphasizes that correct insulin injection techniques and skills are vital for effective diabetes treatment, even among medical workers at the community level.

"How can they instruct patients properly about insulin injection skills if they themselves are ignorant," Ji, head of CDS, says.

To help address these problems, the relevant health authorities and CDS have initiated a series of awareness campaigns like organizing training for medical workers.

With support from international pharmaceutical companies, Lilly and BD, CDS has just issued the nation's first diabetes drug injection technique guidelines.

"The guidelines mainly target medical workers and we are now working on a second one tailored to patients themselves, in simpler terms and with more illustrations," Ji says.

The full version of the guidelines will be available soon on the Chinese Medical Association website (http://www.cma.org.cn/) for free download.

A new book about the life of Coco Chanel published in the United States on Tuesday aims to strengthen claims the French designer collaborated with the Nazis during World War II as a spy code-named "Westminster."

The book, "Sleeping With The Enemy: Coco Chanel's Secret War," by Paris-based American journalist Hal Vaughan, claims that not only was the designer the lover of a German officer, Hans Gunther von Dincklage, which has been well-documented, but they were spies who went on missions to Madrid and Berlin.

In addition, the book claims Chanel was deeply anti-Semitic.

"Vaughn reveals that Chanel was more than just a Nazi sympathizer and collaborator. She was a numbered Nazi agent working for Abwehr, Germany's military intelligence agency," publisher Alfred A. Knopf said in a statement.

But a representative for the Chanel fashion house on Tuesday poured doubt on the book's allegations.

"What's certain is that she had a relationship with a German aristocrat during the War. Clearly it wasn't the best period to have a love story with a German even if Baron von Dincklage was English by his mother and she (Chanel) knew him before the War," the Chanel group said in a statement.

The fashion house also disputed that the designer was anti-Semitic, saying Chanel would not have had Jewish friends or ties with the Rothschild family of financiers if she were.

But the book draws on English, French, German and American archives to claim Chanel, whose menswear-inspired fashions propelled her to become one of the most influential figures in fashion, went on missions with Dincklage and others to help recruit new agents willing to serve Germany.

It gives her Abwehr agent number as F-7124 and code-name as "Westminster," named after the Duke of Westminster with whom she had a love affair. She died in Paris in 1971, age 87.

She has long been speculated about as being a spy, but was released after being questioned about her ties to Nazi Germany by a judge in France. The book prints some excerpts of her court testimony.

She told vogue.co.uk: "I would love to see Kate Middleton carrying Analeena. She is elegant and beautiful and has the style of the Analeena lady."


Lina says when she was creating the bags, she wanted them to be stylish-yet-practical and for women who appreciate luxury.

She said: "I wanted to create a kind of universe of things that are classic, unique, eye catching, yet practical.

"Our customer is the lady that appreciates and understands luxury, diversity and style, the lady that wants to stand out in her own way, a lady who appreciates innovation, craftsmanship, detail and exclusivity."

As well as Dita and Molly being a fan of her designs, Blake Lively's 'Gossip Girl' alter-ego Serena van der Woodsen has also been spotted wearing one and Lina thinks she is a good representative of the brand.

She said: "Serena is elegant, quirky, classy and sexy, and not afraid to experiment in her style, so yes I think she is."

'Delight' at Jennifer's American Idol deal

'American Idol' creator Simon Fuller is "delighted" Jennifer Lopez is returning to judge on the show.

The executive producer explained it is a "positive" move that last year's panellists - Aerosmith frontman Steven Tyler, record producer Randy Jackson and the 'On The Floor' singer - will be rejoining the singing competition for its 11th series.

He said in a statement: "Having both Jennifer and Steven return to 'Idol' in 2012 alongside Randy is such a positive reflection on what was a wonderful and hugely successful season last year, and everyone is delighted."

Earlier this month, Nigel Lythgoe, executive producer of the show, revealed Jennifer and Steven - who replaced Simon Cowell, Kara DioGuardi and Ellen DeGeneres as judges last year - would be rejoining Randy on the programme, and he confirmed Ryan Seacrest would return to present the forthcoming series.

Nigel explained: "I am delighted to say that all three judges, along with the brilliant host of 'American Idol' are back for the next season."

Fox television network has since confirmed the trio have signed up for another series of the talent show, which will begin on January 22.

Jennifer - who recently split from third husband Marc Anthony - previously admitted to being unsure about whether to return as a judge on 'American Idol'.

She said: "I haven't been forced to make a decision and I'm glad about that because, honestly, I'm very on the fence about it."

Will Kate be the next fashion icon?

Nowadays, if there's one woman that you cannot avoid seeing in daily headlines, it is Kate Middleton

Everything makes the news, such as the teapot, T-shirts, and even condoms! And of course, "What will Kate's wedding dress be like?" has become one of the hottest discussed issues thanks to the love of beauty that humans share.

You may be tired of reading about one-angle reports about the bride-to-be, and China Daily website presents a Fashion in politics review, just in time for the royal wedding.

We take you on a visual tour of how some first ladies dress, and examine their fashion codes and impact on fashion, and the political world.
To start, let's look at Kate Middleton, Michelle Obama, and Carla Bruni-Sarkozy.

Kate Middleton: Between Royal and Modern

A recent BBC poll of 2,000 women across the UK shows the public thinks Kate is a bigger fashion icon than Lady Gaga and Cheryl Cole, and another poll conducted by YouGov among 1,000 people 18 and older shows 86 percent of women don't envy Kate Middleton, even though she's pretty.

To define Kate's dressing style, fashionistas find it as hard as Kate herself does. She is at a transforming stage, battling between royal and modern.

Take the tiara, for instance. Some tabloid reports say Kate is considering wearing flowers in her hair on her wedding day, even though she knows "she's risking being blamed as disrespectful to royal tradition."

"The unknown is how far she's going to present herself as absolutely the traditional bride, and how far her designer might collaborate with her just to give her that bit of difference," said Alice Ritchie, of Agence France-Presse.

Fashionistas also kept close tabs on Kate's wedding dress, saying it is so much more than a dress. UK Vogue Editor Alexandra Shulman points out: "The dress carries not only her personal taste, but her commitment to fashion, her attitude to money, especially in the current economic doldrums."


She must have noticed the unspoken fashion mission she carries. She is reported to have inherited Princess Diana's style: the love of blue and the love of wearing a beret.

* A fashion magnet & high-street brands' sales booster *

The 29-year-old has enough magnetism to attract top fashion designers. Speculations on the designer of Kate's wedding dress include Sarah Burton, creative director at Alexander McQueen, and Sophie Cranston, a relative unknown who worked for McQueen before setting up her own label, Libelula.


A Grazia magazine editor said Kate's classic, natural and a bit conservative elegancy have attracted top designers, "Bruce Oldfield, Princess Diana's favorite designer, is said to be making Kate's bride veil, and James Pryce will do her hair, Adele Clarke will make her a pair of shoes."

Whoever the designer will be, "he or she is poised to become the most famous fashion designer on earth overnight, the moment Kate walks down the aisle at Westminster Abbey," said the editor.

Kate worked as an accessories buyer for Jigsaw, a British clothing brand, after graduation from the University of St. Andrews in Scotland. During her time there, she collaborated with jewelry designer Claudia Bradby on a necklace that's now being reissued for $71.

Kate's fashion influence became clear as far back as 2007, when a dress she wore for her 25th birthday (a £40 Topshop tunic) sold out within 24 hours, a Topshop retailer told MailOnline news.

Retailers are reported to be ready to get reproductions out on the streets and the image printed on souvenirs.

"We have to remember that this dress is going to be seen forever. She is our future queen," said Caroline Castigliano, a British wedding dress designer.

* Fashion upgrade *

However,not every fashionista likes Kate's fashion taste. Vivienne Westwood has mentioned the bride-to-be does not share her idea of dressing and that she's not bothered about not having the honor to make Kate's wedding dress.

When Kate was spotted enjoying her last single shopping spree on King's Road in London a week ago, the Telegraph reported that she spent £225 for four size 8 dresses from Warehouse, a mid-market women's fashion chain. Other shops she went to include her favorite, Whistles— and there, her shopping bags reveal the princess-to-be is still in the "way-too-common" fashion stage.

Fashion columnist Liz Jones said, "I like Kate's down-to-earth attitude to fashion, I really do, but a big part of me wishes she would up the ante."
