doctors to drive anti-smoking initiative

Chinese doctors will join the country's anti-smoking campaign and become role models for the country's 350 million smokers under a recent initiative launched in Beijing by the Ministry of Health, China Medical Board (CMB) and the Bill & Melinda Gates Foundation.

The five-year-long China Medical Tobacco Initiative, designed exclusively for Chinese medical professionals, is aimed at building smoke-free campuses and smoke-free hospitals, promoting tobacco control in medical education and improving hospital services for those wanting to quit.

doctors to drive anti-smoking initiative

The ultimate goal of the program is to create a "smoke free China", says CMB President Professor Lincoln Chen.

International evidence shows that the medical profession is a critical and powerful force in tobacco control. In China, however, studies show more than 50 percent of male doctors smoke.

Smoking by doctors not only goes against their "health ambassador" image, but also affects their credibility when they attempt to help patients, says Professor Chen.

Medical professionals, universities and institutions are essential to a campaign aimed at mass behavioral change for the reduction and ultimately the elimination of smoking in China, as in all societies, he adds.

The program, funded by the Gates Foundation, will be executed by CMB working closely with 13 medical universities and their affiliated hospitals in the country.

Under it, all medical universities pledge to exercise the social responsibility of medical schools to safeguard public health; faculty and students undertake not to smoke, not persuade others to smoke and not give cigarettes as presents; and all medical campuses and affiliated hospitals strive to become entirely smoke free.

Tobacco control will be built into the medical-education curriculum and research encouraged.

China is the world's largest tobacco producer and consumer. Currently, it has 350 million smokers, plus 450 million people exposed to second-hand smoke.

Each year, 1 million deaths are caused by smoking-related diseases, according to the Ministry of Health. The Chinese government has signed the WHO Framework Convention on Tobacco Control, and from January 2011 will completely ban smoking in all indoor public areas, indoor work sites, public transportation and all outdoor public facilities.

Swine flu could hit up to 40 percent in US

Swine flu could hit up to 40 percent in US

This Oct. 5, 2007 file photo shows a nurse filling a syringe with a flu vaccination at a CVS pharmacy in the Brooklyn Borough of New York. U.S. health officials say swine flu could strike up to 40 percent of Americans over the next two years and as many as several hundred thousand could die if a vaccine campaign and other measures aren't successful.[Agencies]

ATLANTA – In a disturbing new projection, health officials say up to 40 percent of Americans could get swine flu this year and next and several hundred thousand could die without a successful vaccine campaign and other measures.

The estimates by the Centers for Disease Control and Prevention are roughly twice the number of those who catch flu in a normal season and add greater weight to hurried efforts to get a new vaccine ready for the fall flu season.

Swine flu has already hit the United States harder than any other nation, but it has struck something of a glancing blow that's more surprising than devastating. The virus has killed about 300 Americans and experts believe it has sickened more than 1 million, comparable to a seasonal flu with the weird ability to keep spreading in the summer.

Health officials say flu cases may explode in the fall, when schools open and become germ factories, and the new estimates dramatize the need to have vaccines and other measures in place.

A world health official said the first vaccines are expected in September and October. The United States expects to begin testing on some volunteers in August, with 160 million doses ready in October.

The CDC came up with the new projections for the virus' spread last month, but it was first disclosed in an interview this week with The Associated Press.

The estimates are based on a flu pandemic from 1957, which killed nearly 70,000 in the United States but was not as severe as the infamous Spanish flu pandemic of 1918-19. The number of deaths and illnesses from the new swine flu virus would drop if the pandemic peters out or if efforts to slow its spread are successful, said CDC spokesman Tom Skinner.

"Hopefully, mitigation efforts will have a big impact on future cases," he said. Besides pushing flu shots, health officials might urge measures such as avoiding crowded places, handwashing, cough covering and timely use of medicines like Tamiflu.

Because so many more people are expected to catch the new flu, the number of deaths over two years could range from 90,000 to several hundred thousand, the CDC calculated. Again, that is if a new vaccine and other efforts fail.

In a normal flu season, about 36,000 people die from flu and its complications, according to the American Medical Association. That too is an estimate, because death certificates don't typically list flu as a cause of death. Instead, they attribute a fatality to pneumonia or other complications.

Influenza is notoriously hard to predict, and some experts have shied away from a forecast. At a CDC swine flu briefing Friday, one official declined to answer repeated questions about her agency's own estimate.

"I don't think that influenza and its behavior in the population lends itself very well to these kinds of models," said the official, Dr. Anne Schuchat, who oversees the CDC's flu vaccination programs.

The World Health Organization says as many as 2 billion people could become infected in the next two years — nearly a third of the world population. The estimates look at potential impacts in a two-year period because past flu pandemics have occurred in waves over more than one year.

Swine flu has been an escalating concern in Britain and some other European nations, where the virus' late arrival has grabbed attention and some officials at times have sounded alarmed.

In an interview Friday, the WHO's flu chief told the AP the global epidemic is still in its early stages.

"Even if we have hundreds of thousands of cases or a few millions of cases ... we're relatively early in the pandemic," Keiji Fukuda said at WHO headquarters in Geneva.

The first vaccines are expected in September and October, Fukuda said. Other vaccines won't be ready until well into the flu season when a further dramatic rise in swine flu cases is expected.

First identified in April, swine flu has likely infected more than 1 million Americans, the CDC believes, with many of those suffering mild cases never reported. There have been 302 deaths and nearly 44,000 laboratory-identified cases, according to numbers released Friday morning.

Because the swine flu virus is new, most people haven't developed an immunity to it. So far, most of those who have died from it in the United States have had other health problems, such as asthma.

The virus has caused an unusual number of serious illnesses in teens and young adults; seasonal flu usually is toughest on the elderly and very young children.

Season's menswear go macho and meshy

Season's menswear go macho and meshy

In fashion, the future is clear. Say farewell to fuzzy numbers and hiding behind the corporate veil as fabrics accentuate the obvious for spring. Mesh and transparent looks dominate the runway for the latest menswear 2010 spring collections.

"I really like the concept of transparency with what's going on in this recession," says Italo Zucchelli, Calvin Klein Collection men's creative director. "I saw some X-ray pictures of objects and wanted to translate the concept into clothes."

Zucchelli chose mesh that worked as a network, with netting forming perfect diamonds, all in harmony. A black mesh jacket has a sporty, techno feel, but is actually constructed of starched cotton.

"I wanted to make references to athletic wear, but also make the extreme mesh really open and show it in a way you can understand," he says. "I didn't want to put a guy naked underneath. Realistically, a jacket with open mesh is worn with a T-shirt."

Prada layered mesh upon mesh, capturing the peek-a-boo element of it all. Rick Owens went more literal with transparent fabrics and a bare-chested wet T-shirt look in his collection, while Dior Homme stayed discreet in collared black shirtings almost fit for the office.

"I used transparent fabrics to give the garment a very lightweight, airy feel," says designer Tim Hamilton.

His bare tendencies came in the form of gray and white anoraks reminiscent of a rain-drenched camping trip. Moncler Gamme Bleu took semi-transparent plaid rain suits poolside, as if to say this functional interpretation truly holds water.

LOS ANGELES– Latin singer Marc Anthony has acquired a minority interest in the Miami Dolphins football team, joining entertainers Jimmy Buffett and Gloria Estefan who also are associated with the storied sports franchise.

Anthony, who has sold more than 10 million records in his career and is married to actress and singer Jennifer Lopez, said on Tuesday that he grew up in a sports-oriented family and dreamed of being part of a franchise.

"It's just a dream come true, a once-in-a-lifetime opportunity and a no-brainer," Anthony said in a conference call with reporters.

The Dolphins, whose majority owner and general partner is real estate developer Stephen Ross, played their first game in 1966 and are the only professional football team to have enjoyed a perfect season, winning all their games and the Super Bowl in the 1972/1973 season.

Anthony expects to be an active part of the Dolphins organization working in Miami to drum up community support for the team.

Estefan and her husband, record producer Emilio Estefan, also are minority partners in the team, and "Margaritaville" singer Buffet recently announced a promotional partnership that included renaming Dolphin Stadium to LandShark Stadium, named after Buffett's LandShark lager.

Anthony's involvement with the Dolphins was announced at a news conference in New York City. Financial details were not disclosed.

Obama challenges GOP critics on health care

Obama challenges GOP critics on health care

WASHINGTON – President Barack Obama pushed back hard against Republican critics of his health care overhaul plan Monday, dismissing the "politics of the moment" marked by GOP comparisons of his efforts to socialism.

Struggling to revamp the nation's $2.4 trillion health care system, the president gave ground on his tight timetable for passage of sweeping legislation.

Obama's strong words came just hours after Republicans ratcheted up their criticism of the president and congressional Democrats. Michael Steele, chairman of the Republican Party, likened Obama's plans to socialism and argued that the president, House Speaker Nancy Pelosi and key congressional committee chairmen are part of a "cabal" that wants to implement government-run health care.

The White House also faced troubling news in the latest polling, with approval of Obama's handling of health care slipping.

"We can't afford the politics of delay and defeat when it comes to health care," Obama said after meeting with doctors, nurses and other health care workers at Children's National Medical Center. "Not this time. Not now. There are too many lives and livelihoods at stake."

Without mentioning his critic by name, the president recounted South Carolina Republican Sen. Jim DeMint's comment that stopping Obama's bid for health care overhaul could be the president's "Waterloo," a reference to the site of Napoleon's bitter defeat in 1815.

"This isn't about me," Obama responded. "This isn't about politics. This is about a health care system that is breaking America's families, breaking America's businesses and breaking America's economy."

Striking a more populist tone than in past remarks, the president complained that "health insurance companies and their executives have reaped windfall profits from a broken system."

"Let's fight our way through the politics of the moment," Obama said. "Let's pass reform by the end of this year."

That reflects a shift in a timetable he has stressed repeatedly. Obama had said previously that he wanted the House and Senate to vote on legislation before lawmakers leave town for their August recess, with a comprehensive bill for him to sign in October.

"I want this done now. Now, if there are no deadlines, nothing gets done in this town," Obama told PBS's "The NewsHour." "If somebody comes to me and says 'It's basically done, it's going to spill over by a few days or a week,' you know, that's different."

He said too much of the focus has been on what has not been accomplished instead of on a coalition of health companies, professionals and constituents. Later in the day, aides organized a conference call for Obama to speak with liberal bloggers and rally them behind the White House's broad outline for overhaul.

"One of the things that I know the blogs are best at is debunking myths that can slip through a lot of the traditional media outlets and a lot of the conventional wisdom," he said, according to audio of the call posted on Web sites. "And that is why you are going to play such an important role in our success in the weeks to come."

Steele accused Obama of conducting a risky experiment that will hurt the economy and force millions to drop their current coverage.

"Obama-Pelosi want to start building a colossal, closed health care system where Washington decides. Republicans want and support an open health care system where patients and doctors make the decisions," Steele said in a speech at the National Press Club.

Asked whether Obama's health care plan represented socialism, Steele responded: "Yes. Next question."

Obama has said he does not favor a government-run health care system. Legislation taking shape in the House envisions private insurance companies selling coverage in competition with the government.

The president is struggling to advance his trademark health care proposal after a period of evident progress. Two of three House committees have approved their portions of the bill, while one of two Senate panels have acted. A Washington Post-ABC News survey released Monday shows approval of Obama's handling of health care overhaul slipping below 50 percent for the first time.

The president, who spent most of last week making his plea for health care overhaul, was pressing his case hard again this week, first at the children's hospital, and later this week in a prime-time news conference Wednesday and a town hall in Ohio on Thursday.

Conservative Democrats have raised objections to some elements of the legislation, and Pelosi is floating an idea that could make proposed tax increases more palatable to them. She would like to limit income tax increases to couples making more than $1 million a year and individuals making more than $500,000, Pelosi spokesman Brendan Daly said Monday. The bill passed by the House Ways and Means Committee last week would increase taxes on couples making as little as $350,000 a year and individuals annually making as little as $280,000.

In the Senate, negotiators seeking a bipartisan compromise reported progress Monday. Finance Committee Chairman Max Baucus, D-Mont., said there's tentative agreement on four big policy issues out of a list of about one dozen. He would not elaborate.

Separately, senators are discussing a variation on the idea of taxing high-cost health insurance benefits. The proposal would not raise taxes on individuals and families. Instead, insurers and employers who offer the benefits would pay the tax. Advocates say such a tax would encourage people to be thriftier consumers of health care. Prospects are uncertain.

Obama and Democratic leaders face a new batch of ads.

Republican officials said they were supplementing Steele's speech with a round of television advertising designed to oppose government-run health care. The 30-second commercial, titled "Grand Experiment," criticizes recent government aid to the auto industry and banks as "the biggest spending spree in our history" and warns similarly of "a risky experiment with our health care."

The U.S. Chamber of Commerce, the nation's largest business group, planned to announce ads of its own Tuesday criticizing the government-run insurance proposal, saying it would threaten employer-provided coverage.

R. Bruce Josten, the group's top lobbyist, said the campaign would begin with a $2 million budget and include newspaper and Internet ads, as well as efforts to drum up public support across the country. The ads will appear in Capitol Hill newspapers beginning Tuesday, then in coming days in newspapers in Arkansas, Louisiana, Colorado, Nebraska and other states where lawmakers are wavering.

Citing liberal and labor groups that have run ads criticizing Democrats who have not endorsed the health care effort, Josten said, "It's time to push back a little bit."

Separately, the insurance industry, which challenged then-President Bill Clinton's health care effort in the early 1990s, launched a $1.4 million ad campaign, its first TV ads of this year's health care fight. The multimillion-dollar campaign, being aired nationally on cable stations, restates the industry's support for an overhaul that provides universal coverage and its offer to cover people who are already sick. The ad campaign does not mention the insurers' strong opposition to creating a government-run insurance option.

An official disclosed the cost of the campaign on condition of anonymity, as the numbers have not been made public.

All nations must have access to vaccines, says WHO

The swine flu pandemic has grown "unstoppable" and all nations will need access to vaccines, a WHO official said on Monday, as 12 new deaths were reported and a study raised fresh concerns.

Britain, Brazil, Colombia, Mexico, the Philippines and Thailand all reported deaths on Monday, while Saudi Arabia shut an international school after 20 students were diagnosed with the H1N1 virus.

As the death toll increased, the World Health Organization official said a swine flu vaccine should be available as early as September and all countries would need to be able to protect themselves.

All nations must have access to vaccines, says WHO

A group of vaccination experts concluded after a recent meeting that "the H1N1 pandemic is unstoppable and therefore all countries would need to have access to vaccines," says Marie-Paul Kieny, WHO director on vaccine research.

Health workers should be at the top of the list for vaccination since they will be in high demand as people continue to fall sick, she adds.

Countries would be free to decide on their national priorities, but other groups should include pregnant women and anyone over 6 months old who has chronic health problems, the WHO official says.

Particular attention would have to be paid to children since they are considered "amplifiers" of the spread of the virus, especially when gathered in schools, Kieny adds.

More than 90,000 swine flu cases have been reported worldwide, the most recent WHO numbers show.

While most cases have been considered mild, a study released on Monday said the virus causes more lung damage than ordinary seasonal flu strains but still responds to antiviral drugs.

The worry about the present strain of H1N1 is that it could pick up genes from other flu strains that would enable it to be both highly virulent and contagious, and these warnings are spelt out in the new study.

"Sustained person-to-person transmission might result in the emergence of more pathogenic variants, as observed in the 1918 pandemic virus," it says.

U.S. to spend another $1 billion on flu vaccine

WASHINGTON - The United States will spend another $1 billion on ingredients for an H1N1 vaccine, U.S. Health and Human Services Secretary Kathleen Sebelius said on Sunday.

"There'll be another $1 billion worth of orders placed to get the bulk ingredients for an H1N1 vaccination. Congress has agreed with the president that this is the number one priority, keeping Americans safe and secure," Sebelius said on CNN.

Sebelius has said plans were on track for a mid-October vaccination program, although it was not certain Americans would be offered the vaccine for the so-called swine flu.

"We are aggressively working on, first of all, testing the virus strains to get a vaccination ready. It needs to be safe so testing and clinical trials will start this month. We'll know a lot more by the end of the summer and it needs to be effective," she said.

The World Health Organization may issue guidance as soon as Monday on whether an H1N1 swine flu vaccine will be offered alongside the seasonal flu vaccine.

Vaccine makers Sanofi-Aventis, Novartis, Baxter, GlaxoSmithKline, Solvay and AstraZeneca's MedImmune subsidiary have finished making seasonal flu vaccines for this year.

The U.S. Food and Drug Administration has scheduled a July 23 advisory panel meeting to discuss clinical trials of the vaccines against the H1N1 influenza virus and the U.S. Advisory Committee on Immunization Practice wills meet July 29.

"FDA is working with the scientists at NIH (National Institutes of Health) to make sure that we have a safe and effective strain and then we're getting ready to make sure that we have a vaccination program," Sebelius said.

Health experts estimate at least 1 million people have been infected with H1N1 in the United States, and the U.S. Centers for Disease Control and Prevention has confirmed 211 deaths. It often takes weeks or months to collect data on flu deaths.

About 36,000 people die each year from the seasonal flu in the United States alone, and 250,000 to 500,000 die globally.

Scientists kill cancer cells with "trojan horse"

Scientists kill cancer cells with

A blood sample drawn from cancer patient will be tested to monitor whether chemotherapy has caused red and white blood cell counts to drop too low during a weekly test at Georgetown University Hospital's Lombardi Cancer Center in Washington May 23, 2007.[Agencies]

SYDNEY - Australian scientists have developed a "trojan horse" therapy to combat cancer, using a bacterially-derived nano cell to penetrate and disarm the cancer cell before a second nano cell kills it with chemotherapy drugs.

The "trojan horse" therapy has the potential to directly target cancer cells with chemotherapy, rather than the current treatment that sees chemotherapy drugs injected into a cancer patient and attacking both cancer and healthy cells.

Sydney scientists Dr Jennifer MacDiarmid and Dr Himanshu Brahmbhatt, who formed EnGenelC Pty Ltd in 2001, said they had achieved 100 percent survival in mice with human cancer cells by using the "trojan horse" therapy in the past two years.

The scientists plan to start human clinical trials in the coming months. Human trials of the cell delivery system will start next week at the Peter MacCullum Cancer Center at the Royal Melbourne Hospital and The Austin at the University of Melbourne.

The therapy, published in the latest Nature Biotechnology journal, sees mini-cells called EDVs (EnGenelC Delivery Vehicle) attach and enter the cancer cell.

The first wave of mini-cells release ribonucleic acid molecules, called siRNA, which switch off the production of proteins that make the cancer cell resistant to chemotherapy.

A second wave of EDV cells is then accepted by the cancer cell and releases chemotherapy drugs, killing the cancer cell.

"The beauty is that our EDVs operate like 'Trojan Horses' They arrive at the gates of the affected cells and are always allowed in," said MacDiarmid.

"We are playing the rogue cells at their own game. They switch-on the gene to produce the protein to resist drugs, and we are switching-off the gene which, in turn, enables the drugs to enter."

DISARMING TUMOUR CELLS

RNA interference, or RNAi, is designed to silence genes responsible for producing disease-causing proteins and is one of the hottest areas of biotechnology research. RNA was the basis of the 2006 Nobel Prize in medicine.

Dozens of biotechnology companies are looking for ways to manipulate RNA to block genes that produce disease-causing proteins involved in cancer, blindness or AIDS.

Brahmbhatt said that after treatment with conventional drug therapy, a large number of cancer cells die but a small percentage of the cells can produce proteins that make cancer cells resistant to chemotherapeutic drugs.

"Consequently, follow-up drug treatments can fail. The tumors thus become untreatable and continue to flourish, ultimately killing the patient," said Brahmbhatt.

"We want to be part of moving toward a time when cancers can be managed as a chronic disease rather than being regarded as a death sentence," he said.

Therapy of the future

Therapy of the future

American medical students learn cupping from Chinese doctors at a Shanghai hospital.

In recent years, cupping has returned as a cure within the framework of so-called "non-traditional medicine". However, its controversial methods of practice take different forms.

Doaa Mahgoub, a physician at the Emergency Unit of King Abdulaziz University hospital in Saudi Arabia's Western city of Jeddah, says that sucking impure blood follows the same traditional methods used in Saudi Arabia and other Arab and Islamic countries thousands of years ago.

"East Asian countries including China and Japan practice cupping in a different way," Mahgoub says.

She adds that dry cupping or the use of air cups is widespread in mountainous areas where people are liable to suffer clotting due to high blood density.

Cupping, known in the past as blood sacrifice, was a means of protecting those people from blood clotting, as recorded in medical registers of the Arabs more than 1,400 years ago.

Mahgoub pointed out that this primitive method is still practiced in some areas in Saudi Arabia. It depends on using elephant horns imported from India.

There are three types of cupping according to Mahgoub: dry cupping, impure blood sucking and massage cupping.

She says that Sheikh Ahmad Hefny is accredited with having revived the third type of cupping in Egypt through strict measures established to protect both the patient and the cupper. Disposable instruments are used each time.

Sheikh Hefny teaches this therapeutic practice to about 5,000 doctors and 3,500 practitioners in different Arab countries.

The modern practice of cupping is described as "entirely safe and non-feared when compared with the old non-disposable brass cups" by assistant professor of surgery and endoscopy Adel al-Gohary.

"Thus, risks of catching hepatitis or AIDS have largely disappeared," adds al-Gohary, who is also an expert in cupping at the same hospital.

Al-Gohary explains that sterilizing applied instruments and placing honey on the blood-sucking areas and other preventive measures were not taken in the past. He added that this helped protect patients from catching hepatitis or AIDS.

"A cupper avoids sucking blood from veins and arteries and concentrates on the cellular fluid to reduce negative effects and keep side effects to the minimum level," he adds.

Al-Gohary says that cupping proved to be very effective in curing chronic diseases, neck and back pain, headaches, ophthalmic diseases, abdomen pain, diabetes, digestive diseases and hypertension.

It also proved effective in increasing white blood cells as well as anti-bacterial and anti-viral immunity bodies.

More than 30 women, who suffered from back pain after giving birth, were treated by gynaecologist Haifaa Mazhar using cupping. While Hany Hashem Moquebel, who works as an engineer in Jeddah, says that cupping was the best remedy to cure his hypertension symptoms.

Hassan Amin Sembawa, a specialist in emergency medicine and holder of an Arab Jordanian fellowship, says that traditional methods of cupping have largely developed as preventive measures were being taken.

He adds that cupping is very much in demand after being proven effective in treating drug addiction.

"Cupping is found in Chinese and Indian medicine as one of the most effective non-traditional therapies," Sembawa says, pointing out the importance of Syrian research studies which were carried out in this field 100 years ago.
Sembawa says that there was an ancient belief that cupping strengthened memory and that some Muslim scholars, including Avicenna and Ahmad Ben Hanbal, had used cupping to help remember forgotten information.

The dean of the faculty of medicine in King Abdulaziz University Adnan al-Mazroue echoes Sembawa's words. He says that cupping was an essential part of ancient Arab medicine

"No one can deny its several benefits as part of Eastern integrated medicine, despite the lack of scientific experiments and scientifically proven facts," says al-Mazroue, who also hailed the role of King Abdulaziz University in carrying out scientific research on cupping to establish references in this domain.

Amin Youssef, a gynaecologist, says that he underwent physiotherapy for his left shoulder. Finally, his pain was relieved when he was treated with cupping.

Youssef argues that cupping is now used in the treatment of gymnasts in the United States and many European countries as it has proved effective in the treatment of ruptured muscles, ligaments and headaches.

Although pharmaceutical companies feared that cupping would affect their sales adversely, particularly of sedatives, its high cost makes it inaccessible.

It costs between 400 and 500 riyals ($107-133) in Riyadh, and between 150 and 300 in west Hejaz, where it is more popular.

Some believe that the medical debate on the use of acupuncture and cupping weighs in favor of cupping as the effect appears more rapid. Treatment using cupping takes about 30 minutes in most cases, while acupuncture treatment takes between seven and 10 days to carry out seven sessions (each session takes 40 minutes).
